I, the undersigned parent or legal guardian of the registered minor participant, acknowledge that participation in the GFiber Youth Training Camp involves physical activity and inherent risks, including but not limited to injury, illness, or accident.
I hereby voluntarily assume all risks associated with my child’s participation and release, waive, and discharge Google Fiber, Kingdom Culture Agency, Vertical Gyms, event partners, sponsors, coaches, volunteers, staff, and all affiliated entities from any and all claims, liabilities, or causes of action arising from participation in the event, to the fullest extent permitted under Tennessee law.
I certify that my child is physically able to participate and has no medical conditions that would prevent safe participation. I authorize event staff to obtain emergency medical treatment if deemed necessary.
